For Graduates

Various certificates can be applied at the office or by mail. Please show us your ID card that verifies your identity. It takes three days to issue a certificate after receiving an application for it. (Excluding Saturdays, Sundays, national holidays, and during summer and winter vacations.) In case by mail, the number of days required for mailing is not included.

Please fill out the "Application for Issuance of Certificate" form at the office on the 2nd floor during the office hours.

◆What you need when applying◆
Identification card that verifies your identity (driver's license, passport, residence card or health insurance card)
Issuance fee: 300 yen per copy

Please print out the "Application for Certificate Issuance" form below and fill it out. Enclose the postage stamps as the issuance fee (300 yen per copy x number of copies required) and a copy of your ID (driver's license, passport, residence card or health insurance card), and mail it to the address below.
